STUNT group LESSON REQUEST FORM

• PLEASE REFERENCE THE CURRENT SCHEDULE OF AVAILABILITY THROUGH YOUR iCLASS PARENT PORTAL PRIOR TO SUBMITTING THIS FORM SO YOU KNOW WHAT IS POSSIBLY AVAILABLE AS A STUNT GROUP LESSON.

• A Stunt GROUP lesson is ONE Coach instructing a full stunt group (4 athletes). All 4 athletes will need to be booked into the lesson, you will indicate their names below, and each individual athlete’s account will be charged for their spot in the lesson.

  • You MUST have 4 athletes to book this lesson type, and each athlete’s account will be charged $15 each, equaling the $60 per half hour lesson.

• This form is to be submitted if you are looking to book a one-time stunt GROUP lesson for your athlete.

• If you are looking to book multiple stunt group lessons, you will submit a separate request for each lesson.

• Please allow 24-48 business hours from the date of submission for a response about your request.

• Please be as thorough as possible with your request or you will receive a response that it cannot be fulfilled.
